Read the site criteria on the application form carefully. If your property qualifies, please print out the application, complete it, and have the property owner sign it.

Then, please deliver the form to the NLNA Office, 700 N. 3rd Street, 19123
by Friday, May 1. ( There's a mail slot in the Fairmount Ave. side door)
